ADMISSIONS AND DISCHARGE INFORMATION
MAIN CAMPUS, JARRATT, VIRIGNIA
PRE-PLACEMENT AND ADMISSIONS PROCESS
New residents may be admitted Monday through Thursday, 9:00 AM until 4:00 PM and Friday from 9:00 AM until 1:00 PM. (After-hours admissions may be arranged with advanced notice).
The following documents are required at the time of admission:
- Birth Certificate
- Social Security Card
- Immunization Record
- Custody Order, if applicable
- Court Order, if applicable
- Medicaid Card, if applicable
- Copy of third party insurance card/information
- Copy of juvenile court probation rules, if applicable
- History of any criminal charges, dates and disposition of each charge
- Medical history
- Last Dental exam and dental report
- Physical exam including any chronic illnesses, previous history of serious injury, illness, infectious/communicable diseases and allergies within 30 days of admission
- Social history, which includes an update of the current situation
- History of out-of-home placements with dates and places
- Recent Psychological Evaluation with IQ scores
- School Records
- Current IEP, if child requires special education resource or classes
- The name and address of the last school attended and the dates of attendance
- Discharge summary from prior hospitalizations
- Discharge summary from prior residential placements, including group homes
A Jackson-Feild Homes’ Placement Agreement must be signed by the legal guardian and placing agency on or prior to the day of placement. A resident may not be admitted without legal authorization and signature for placement.
Admission Day
Jackson-Feild Homes’ main facility is a “campus” environment located in a rural area. A new resident enters the facility via the Admissions Office where she will meet with the Admissions Coordinator and her assigned case manager. Once she has been cleared physically by nursing, she will be escorted to her assigned cottage where she will receive an orientation to the program, be introduced to staff and residents, and complete all her admitting tasks, such as a personal inventory of items.
