REFERRAL PROCEDURES

To make a referral, you may contact our Admissions Coordinator, Kate Turner @ 434-634-3217. All referral packets may be submitted by one of the following methods:

The following information must be included in the referral packet:

1. Application Form (available online at this website)
2. Social history, which includes an update of the individual’s current situation
3. Medical History (Immunization Record & most recent Physical)
4. Psychological Evaluation or DSM-IV Diagnosis (if applicable)
5. Education History (most recent report card & IEP, if applicable)
6. Discharge Summaries from Previous Placements in past two years
7. History of any criminal charges, dates and disposition of each charge

Once the application is complete, referral information will be reviewed by Jackson-Feild Homes’ Admission/Discharge Committee within 24 business hours. If the referral meets the program admission criteria and space is available, an admission date will be scheduled. If an interview is deemed necessary after review of the referral packet, our staff will schedule it as quickly as possible. Interviews may be held at Jackson-Feild Homes or at the child’s current placement. A pre-admission tour and visit is welcome but not required.

The Admissions/Discharge Committee reserves the right to require a face-to-face initial interview prior to making a decision if the referral packet is not self-sufficient for a thorough assessment or if the packet contains conflicting information about the candidate and it is difficult to assess whether she meets the criteria or not.
